The U.S Citizenship and Immigration Services (USCIS), the government agency that administers lawful immigration to the United States. We are pleased to offer the immigration physicals required by the USCIS. Individuals seeking Green Card or applying for citizenship have to take their physicals conducted by the designated civil surgeon. I-693 Paper containing the results of the physicals will be given to the applicant, which has to be returned to USCIS. The Immigration practice can be finished in 3 visits and that depends on whether a tuberculosis test (PPD) is required.

- Tuberculosis test for applicants 2 and older. It requires another visit to the office, chest X-ray will be required if the test result is positive.
- Completed and sealed I-693 paper, also the copies of the laboratory testing.
Applicants have to produce valid, government issued Id like Passport, Driver’s license, Medical or Immunization records, any mental health records and Pre-completed I-693 immigration physical form (check if the form is up to date) during the examination.
Once the immigration physical examination is complete, we will provide you the sealed I-693 form, which has to be presented to the Department of Homeland Security and U.S. Citizen and Immigration Services as is. Also, the copy of the blood tests and the physical examination will be provided to the applicant. The immigration physical examination will be valid for one year from the date of examination.
